Accident Disclaimer and Waiver of Liability
Padel Cornwall (Thomas Hutchins trading as Padel Cornwall) - Waiver of Liability and Consent Form
By participating in any beach tennis activities organised by Padel Cornwall, I, the undersigned, acknowledge and agree to the following terms:
Assumption of Risk: I understand that playing beach tennis involves physical activity and inherent risks, including but not limited to the possibility of personal injury. I voluntarily accept and assume all risks associated with participating in beach tennis activities, whether they are known or unknown, foreseeable or unforeseeable, including risks of injury, accident, or other harm. I agree to participate in the activities at my own risk.
No Financial Compensation: I acknowledge that all beach tennis activities are provided free of charge by Padel Cornwall. I am not entitled to any compensation or reimbursement for any injury or harm sustained during participation.
First Aid Kit: A first aid kit is available at the event location. However, I understand that the availability of such materials does not guarantee immediate or adequate treatment in the event of injury, and I am solely responsible for seeking medical attention if necessary.
Release of Liability: I hereby release, waive, and discharge Padel Cornwall (Thomas Hutchins trading as Padel Cornwall), its members, organisers, affiliates, sponsors, volunteers, and any other individuals associated with the event from any and all liability for injuries, damages, or losses arising out of my participation in the beach tennis activities.
Consent for Photography and Videography: I understand that photographs and videos may be taken during the beach tennis activities. By participating, I grant my full consent to the use of my image, voice, and likeness in any photos, videos, or recordings taken during the events. I acknowledge that these materials may be used for promotional, marketing, and social media purposes by Padel Cornwall, without compensation to me.
In the event of an emergency, I authorise the Padel Cornwall organisers or volunteers to seek medical assistance on my behalf if necessary, understanding that any costs for medical treatment will be my responsibility.
I confirm that I am in good health and capable of participating in the beach tennis activities. I acknowledge that it is my responsibility to disclose any pre-existing medical conditions that may affect my ability to participate, such as heart problems, asthma, or any other conditions that may require special consideration. I agree to inform the organisers in advance of any such conditions and understand that I may need to provide a doctor's note or clearance to participate in the activities.
Membership and Email Updates: By participating in any beach tennis activities, I agree to join Padel Cornwall as a member. As a member, I will receive email updates regarding future events, offers, and promotions. I understand that I can unsubscribe from these emails at any time by following the unsubscribe instructions in the emails or by contacting Padel Cornwall directly.
Padel Cornwall is committed to protecting your personal data and complying with the General Data Protection Regulation (GDPR).
The personal information I provide, including my name, email address, and any other details, will be used for the purpose of managing my participation in beach tennis activities, communication about events, and membership management.
Padel Cornwall will not share my personal data with third parties without my consent, except where required by law or to facilitate event-related activities.
I have the right to request access to the personal data held about me, request correction of any inaccuracies, and request the deletion of my data in accordance with GDPR.
I can contact Padel Cornwall at any time to withdraw my consent, update my details, or request further information about how my personal data is being used.

By joining as a member and therefore consenting to this waiver, I confirm that I have read and fully understood the above terms and agree to abide by them. I acknowledge that I am voluntarily assuming all risks involved in participating in beach tennis activities and provide my informed consent for the use of my image in marketing materials. I also acknowledge that my personal data will be handled in compliance with GDPR.
If I am joining on behalf of a minor as a responsible parent/guardian, I assume full responsibility for this minor and give consent on their behalf to this waiver.
